Second Valid Passport Requirements

You can request a second valid passport using Form DS-82 if your most recent passport meets all five of these requirements:
  • Is being submitted with your application
  • Is undamaged (other than normal “wear and tear”)
  • Was issued when you were age 16 or older
  • Was issued within the last 15 years
  • Was issued in your current name (or you can document your name change with an original or certified copy of your marriage certificate, divorce decree, or court order)
  • You plan to visit a country that will deny a visa due to the fact that your passport contains markings or visas showing you traveled to certain other countries.
  • You need a second passport for immediate travel because of delays in getting a travel visa or some other foreign governmental process that required you to submit your passport.  
If any of the above statements do not apply to you, you must Apply in Person using form DS-11.

Second Passport Required Documents to Expedite Your Application

Gather your undamaged current or expired passport.


1. Travel Itineraries from online travel source or travel agent which show overlapping international travels requiring your issuance of a second passport. 


2. Letter on company letterhead detailing the purpose of travel, proposed international destinations and expected dates of travel.  The letter must be signed by someone other than the traveler.  

  • Complete the Application for Passport Renewal DS-82 form online.
  • Print out the application on two separate pages

Complete Letter of Authorization

  • Complete Letter of Authorization: Please print out the authorization form, sign and date it.
  • (A letter of authorization will be emailed to you once you place your order.) The  courier company will be the name of the company listed at the top of the form.
  • Gather your undamaged current or expired passport.

Passport Photograph

Take One Passport Photograph: (Can be taken at CVS, Walgreens, FedEx Kinkos)

  1. Must be in color
  2. Must be 2 x 2 inches in size
  3. Taken within the last 6 months
  4. You can not wear eyeglasses in your photograph

Print out Proof of Departure

Print out your Proof of Departure:  This will be your airline ticket, travel itinerary, cruise itinerary, bus ticket
