New Passport Requirements

You must apply in person using Form DS-11 if at least one of the following is true:
  • You are applying for your first U.S. passport
  • You are under age 16
  • Your previous U.S. passport was issued when you were under age 16
  • Your previous U.S. passport was lost, stolen, or damaged
  • Your previous U.S. passport was issued more than 15 years ago

If none of the above statements apply to you, you may be eligible to Renew by Mail using Form DS-82.

Complete the Application for New Passport  DS-11 form online.  Print out the application on two separate pages and head  to the nearest  Acceptance Agency in your area.

All persons requesting a new passport must appear at an Acceptance Agency to have his/her identity confirmed.  The Acceptance Agency will charge a fee of $35.00 for this service.  Payment for this service is separate from payment to the U.S. Department of State.

Please ensure to take a check or money order made payable to the U.S, Department of State for payment of the State Department fee(s), no other forms of payment are acceptable.   Temporary checks, counter checks or credit card checks are not acceptable.

Please ensure that when you arrive at the Acceptance Agent you have in your possession:

1)  Typed DS-11 form- Do not sign until told to do so

2)  Birth Certificate/Birth Abroad Certificate or Naturalization Document

3)  One Passport Photo

4)  Completed Letter of Authorization

5)  Travel Itinerary/Confirmation or Business Letter

6)  State Issued Identification/Drivers License or Military Identification Card

7)  Check or Money Order for payment of fees

  • Complete Letter of Authorization: Please print out the authorization form, sign and date it.
  • (A letter of authorization will be emailed to you once you place your order.) The  courier company will be the name of the company listed at the top of the form.

Take One Passport Photograph: (Can be taken at CVS, Walgreens, FedEx Kinkos)

  1. Must be in color
  2. Must be 2 x 2 inches in size
  3. Taken within the last 6 months
  4. You can not wear eyeglasses in your photograph

Print out Proof of Departure

Print out your Proof of Departure:  This will be your airline ticket, travel itinerary, cruise itinerary, bus ticket

Letter on company letterhead detailing: the purpose of travel, proposed international destination(s) and expected date(s) of travel.  

The letter must be signed by someone other than the traveler.         

                                                                                      

(In order to expedite a passport you must be Traveling within 14 days of the date you submit your passport request or if a travel visa is required 30 days from the date of submission.)

For Minor Passports

For minor passports, both parents must appear with children 15 years and younger.   If both parents can not appear, the non-appearing parent must provide a notarized DS-Form 3053 and a copy of his/her drivers' license front and back or passport information copy.
